INFORMAL
If you have a comment about your experience (good or bad) at the
Forum Health Centre, then please let us know - either directly with
the doctor, nurse or other health care professional - or with any
of the receptionists.
You can also speak with the Reception Manager or Practice Manager
- just ask at reception.
Any comments you make will be treated in confidence and will not
appear on your medical notes nor affect you treatment and care as
a patient.
We also welcome praise where it is deserved - it raises staff morale
considerably when their efforts are appreciated.
