Registering as a Patient

TO REGISTER AS A PATIENT - you will need to live within our "catchment" area - SEE MAP.
We can only accept new patients who wish to register with us if they live within the boundary area marked on this map. Patients registered with us who move outside this area will be asked to register with another practice.

Bring your NHS medical card to the surgery - make sure your address is correct (alter your address if you have moved), INCLUDE YOUR CONTACT TELEPHONE NUMBERS and make sure you've signed the card on Page 3;

OR

Complete Form GMS 1 (obtainable from any doctor's Surgery). The person registering with the surgery needs to sign this form (parent/guardian signs on behalf of children under the age of 16 years). You cannot sign on behalf of another adult person unless you provide us with proof of power of attorney. We shall require proof of identity (e.g. passport or photo driving licence) and address (e.g. a recent utility bill)

Each person registering with the surgery is also required to complete a Health Questionnaire and will be invited to have a new patient medical with one of our nursing staff who will ask you to also provide a specimen of urine.

This practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Pateints are registered with The Practice which means that you can see any of our doctors or other health practictioners. However you can state a preference of a usual doctor.
